To be considered for Train Kentucky’s Targeted Case Management course(s), complete the following form. This form will collect the necessary details to process your application, including your name, contact details, payment preference, and preferred course dates.

Once your application has been reviewed and approved, you will receive an email from Train Kentucky that requests that you set a password for your new account that will be created using the email address provided in this form. Additionally, that same email will contain a link to complete your course purchase, which will finalize your enrollment.
